forwarding by email, a cheque for the entry fee made payable to Twin Centre Regional Silver Stick must be sent by mail with
the Application. Entries are accepted on a first received basis when the funds are received.
Please ensure application form is fully completed (rosters may be submitted later, but not later than December
30, 2007). Please ensure your team contact has submitted an email address, as all correspondence will be done by email.
